Reporting a Concern or Injury
Subiaco Lacrosse Club’s policies and procedures are designed to ensure the game is conducted safely, fairly and respectfully, and that all participants uphold positive and appropriate behaviour.

As part of our shared responsibility to protect everyone involved in the club, this page is used to report injuries and any behaviour that may breach club policies, so appropriate action and support can be provided where necessary.
Reporting an Injury
Click on the below link and download a copy of the injury report form. Fill out all of the details as soon as possible after an injury has occurred and email a copy to subilaxpres@gmail.com.
This form will be stored securely and only shared with authorised club personnel for safety, welfare, and insurance purposes.
